The top tier of the register — by hand, Aura reviews every application herself. We verify five things every legitimate med spa is required to have but never has to show the public: your medical director on the state board, your patient-specific order protocol, your malpractice insurance, your injector credentials, and your attestation that nothing’s slipped.

We accept document links (Google Drive, Dropbox, your own hosting). We do not upload files. Make sure the link is accessible to anyone with the URL — Aura will click through. Requirements vary by state; this form is set for California by default.

We verify license status on the state medical board. Active license, no recent disciplinary actions, name matches what you provide here.

III · Patient-Specific Order protocol

A sample (redact patient info) or a copy of your standardized procedures document. Required in CA as of Jan 1, 2026 — replaces broad standing orders.

IV · Malpractice insurance

Current certificate of insurance. Coverage limits must reasonably cover the procedures you perform.

V · Injector roster

A spreadsheet or PDF with every person who performs medical procedures at your spa — their name, license number, and the procedures they perform. We confirm each license is active and the procedures fall within their legal scope.

VI · Attestation

By signing below, you affirm everything above is accurate as of today. Aura re-verifies every 90 days; if anything material changes, you commit to telling us within 14 days.
